This includes educating staff on the role of the social services The Social Worker Assistant participates in the development of a written, interdisciplinary plan of care for each patient/resident to address social services needs The Social Worker Assistant hold meetings with patients/resident, families, and significant others as requested, to provide support and information to improve quality of life The Social Worker Assistant performs patient/resident discharge planning activities such as family/patient education, nursing center documentation, and make referrals to appropriate agencies and suppliers.
