Job Title: Medical Social Worker (MSW) - Home Care

Job Type: Full Time

Work Location: Philadelphia County (In-Person)

Join our team as a Medical Social Worker (MSW) and provide top-tier home care services. Leverage your social work skills to complete social evaluations and provides information and referral services for patients and families/caregivers regarding practical and environmental needs. Collaborate with physicians and community health professionals to deliver comprehensive care.

Responsibilities:

Assesses the psychosocial status of patients related to the patient's illness and environment and communicates findings to the registered nurse.
Provides information to patients or families/caregivers and community agencies.
Assists physician and other team members in understanding significant social and emotional factors related to health problems
Maintains collaborative relationships with organization personnel to support patient care.
Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.

Qualifications:

Graduate of a master's program in social work accredited by the Council on Social Work Education.
Minimum of one (1) years' experience in health care.
Experience in a home health care preferred.
Licensed driver with a reliable automobile
MSW License in the State of PA- PREFERRED
